Claims for Duties Performed During Examinations
Please READ the following carefully
This form is to be used by persons who have completed work for the examinations unit in the capacity of Supervisor, Invigilator, Reader/Writer/Interpreter, Moderator, Model.
There are five (5) sections in this form. The mandatory sections are section A and section E. Please pay special attention to the notes from Section B, C and D.
Section A - Claimant Personal Details
Ensure you complete all the fields which are required. They are denoted with a *
Section B - Details for Supervisors, Invigilators and Examiners
This section is for persons who perform duties such as Supervisors, Assistant Supervisors, Invigilators, Examiners for Human Ecology. These duties are claimed by the Full Day or Half Day.
Note: Supervisors, Assistant Supervisors and invigilators who work more than (3) hrs are paid as a whole day.
Section C - Details on Teachers who conduct or setup for Practicals
Teachers who setup for practicals are paid $3.12 per candidate.
Note: For Tax purposes, teachers who conduct or setup practicals at the same centre will share a total of $140.00. 'Total Personnel' must reflect the total number of teachers who setup or conduct practicals at the same centre.
Section D - Details on persons who perform duties by the hour or passage
This section is for persons who perform duties such as External Examiners, Oral Examiners, Reader/Writer/Interpreter, Art Models, persons who perform Dictation.
